What would you do if disaster struck your community and you were incapacitated? Who would care for your dog? What if you were in a car accident while going out with your dog?

The Jacksonville Dog Scouts are pleased to offer the Disaster Preparedness Record as a free service for our members. This web based system provides one location to store your dog's medical information, veterinarian contact information, emergency contact information for friends who can care for your dog in an emergency, vaccination records, photographs to aid in identifying your dog if you are separated, and more! The system also creates a printable report which can be stored in your car glove box, a fire-safe box in your home, and at your emergency contacts' homes giving you and your contacts' the tools needed to make sure your loved one is safe in an emergency.
